| 内容記述 | 医療安全に関連したシステム・機器等のICT関連機器を中心とした医療安全上の問題を把握することを目的とし、具体的には「患者認証システム」「転倒転落防止機器」「インシデント報告システム」の3項目の効果と課題について、関東広域圏の500床以上の医療施設で、アンケート調査の協力を得た28施設の医療安全管理者に紙面での調査を行った。 その結果、「患者認証システム」は、8割の施設が導入しており、バーコード認証が主流で、導入している施設すべてが輸液輸血投与時に利用していた。導入効果は、間違い未然防止、誤認インシデント報告の減少が、課題は、機器の不足や操作性等の業務負荷によりシステムの認証を利用しない傾向がみられることであった。「転倒転落防止機器」では、対象施設の総所有数が総ベッド数に対して8％という実態が明らかになった。医療安全管理者の満足度も低く、病床数が多い施設ほど現状に対して不満足である傾向がみられた。効果としては、迅速な危険感知が挙げられ、課題としては、機種の選択の難しさ、機器の不足が認められた。「インシデント報告システム」は、8割の施設が導入していた。集計機能、インタラクティブ性など実装機能が向上しているが、職員間での報告格差の問題は是正されていない現状が認められた。 本調査においてICTの活用における医療安全という視点からの現状が把握され、投薬認証を含めた患者認証システムの拡充、転倒転落機器の充足や効果的な管理、基礎教育における医療安全教育充実及び医療安全管理者への主に医療安全教育に対する支援の必要性が示唆された。 Medical safety manager of medical facilities with more than 500 beds were surveyed using a questionnaire about the effects and challenges of using medical safety related ICT. As a result, the effects of "Patient authentication system" were accident prevention and decrease in misidentification incident reports, while the issue was that the authentication was not used. As to "Fall prevention equipments", it was found out that the total number which they possessed was 8% of the total number of their beds. The effect was quick perception of danger, and the issues were difficulty in choosing models and shortage of equipments. The functions implemented in "Incident report system", such as summary/report and interactivity, have been improved, but the gaps of reports among the staff were not corrected. This survey let us figure out the reality of ICT utilization from the view point of medical safety. It was suggested that collecting epidemiological information, supporting medical safety officers and improving education on medical safety as a part of basic education are necessary. |
